Case Management

United Family Recovery, LLC. offers case management services to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting individual needs. Case management services may include interactions with family members, other individuals, or entities.

Providing Management And Support

United Family Recovery, LLC provides substance use disorder case management services that follow the guidelines and stipulations in accordance with ORC code 5122-29-13 for SUD case management.

All SUD Case Management Services will be provided as determined necessary in the case management needs assessment.

Help Where You Need It

Coordinating assessments, treatment planning and crisis
intervention services.;
Providing training and facilitating linkages for the use of
community resources.
Monitoring service delivery.
Obtaining or assisting individuals in obtaining
necessary services.
Assisting individuals in performing daily living activities.
