United Family Recovery understands that effective treatment begins with an accurate assessment and diagnosis. Our clinicians use effective assessment tools that are age, gender, and culturally appropriate in order to obtain the necessary information for adequate treatment

Providing Individualized Assessment

The agency uses various tools to complete initial and comprehensive assessments according to the prevailing standards of care.

United Family Recovery uses instruments and assessment tools that cover substance-related disorders and the most commonly occurring mental health conditions.

These resources include a brief assessment as well as comprehensive diagnostic interviews.

UFR may use but is not limited to, the following instruments as needed in order to support our organization with the necessary fidelity to ensure we are accurately assessing the individualized needs of the program participants:

  • Addiction Severity Index (ASI)

  • CANS – Trauma Comprehensive (CANS-Trauma)

  • Clinical Institute Withdrawal Assessment of Alcohol Scale, Revised (CIWA-Ar)

  • Generalized Anxiety Disorder-7 (GAD-7)

  • Clinical Opiate Withdrawal Scale (COWS)

  • Columbia – Suicide Severity Rating Scale with Risk Scale (CSSRS)

  • Depression Anxiety Stress Scale (DASS21)

  • DMajor Depression Inventory (MDI)

  • Patient Health Questionnaire (PHQ-9)

  • PTSD Checklist for DSM-5 (PCL-5)

Assessments Include:

  • A clinical comprehensive evaluation

  • Determines diagnosis

  • Identifies treatment needs

  • Establishes Initial Treatment Plan (ISP) including necessary referrals

  • Determines level of care
